2 Dead in Violent Dana Point Crash

A four vehicle car accident on Coast Highway killed two people and hospitalized three more.

The accident took place on Sunday afternoon along the Capistrano Beach stretch of Coast Highway between Beach Road and Camino Capistrano. It was reported at 1:21p.m. There were four cars involved in the accident: a Volkswagen Jetta, Mini Cooper, Honda Civic, and a passenger van.

Investigators believe that the Volkswagen and the Mini Cooper were heading northbound on Coast Highway. The van and the Honda were heading southbound. Investigators believe the Volkswagen may have tried to pass the Mini Cooper, which may have contributed to the collision. A student driver was driving the Mini Cooper.

The Volkswagen Jetta was split in half in the accident. Pieces of the Jetta were strewn across the pavement. This vehicle sustained the worst damage in the accident.  Its occupants, a 43 year-old man and a 32 year-old woman died in the accident. Paramedics reported to the scene and found both victims dead. The two victims were a couple. There were no other victims in the car. The two victims are survived by a 10 month old daughter who was with her grandmother at the time of the accident. Names of these victims have not been released.

The van flipped on its side, leaving a 50 year-old woman and a 49 year-old man. One 53 year-old woman in the Civic was injured. The student driver, a 15 year-old boy, and instructor were not seriously injured in the accident. All survivors sustained moderate and non-threatening injuries. Survivors were treated at Mission Hospital in Mission Viejo. The student driver and instructor were treated by paramedics at the scene for mild injuries.

Coast Highway was closed between Camino Capistrano and Palisades Drive so that investigators could examine the scene and clean up the accident. It was later reopened at around 9 p.m.

Investigators are still looking into the cause of the accident. The Orange County Sheriff Investigators are working diligently to investigate the accident. Speed may have been a factor. There was no reason to believe that alcohol or drugs contributed to the accident.

8 Vehicle Fatal Fullerton Crash

The fatal collision took place at around 4 p.m. on Thursday at Harbor Boulevard and Valencia Mesa Drive.

One person died in the accident. Five more people were injured. Officers reported to the scene at around 4 p.m. One man was found dead at the scene. Several people had to be taken to hospitals. They were taken to Western Medical Center Santa Ana and UCI Medical Center in Orange.  The accident took place near St. Jude Medical Center. Some victims were taken to St. Jude Medical Center.

Investigators believe the accident may have been caused by the driver of a yellow Ford F-150. The driver was driving on the wrong side of the road on Harbor Boulevard at a high rate of speed. According to witnesses, it seemed like the driver was intentionally driving on the wrong side of the road. The driver plowed into cars waiting at a red light. There were no skid marks or break marks, meaning the driver did not try to stop.

The impact caused the truck to catapult onto other vehicles. The driver died on impact. This collision resulted in a deadly pile up involving seven additional vehicles. Luckily, the collision took place in front of St. Jude Medical Center. Medical emergency professionals were able to report to the scene quickly. Injuries could have been more severe if the collision had taken place further away from the hospital. One crash victim was even able to walk to the emergency room. Witnesses also jumped into action and helped the victims of the crash. The current conditions of the victims are not known.

One of the victims was a paraplegic. None of the survivors sustained life-threatening injuries. Investigation into the collision is still ongoing. It is not clear whether drugs or alcohol were involved in the collision. The area where the crash occurred was closed to traffic for several hours.

The male driver has been identified as Adrian Davis of Brea. He was 57. The collision took place 6 days before his 58th Birthday. An autopsy will be performed on Adrian Davis to see if drugs or alcohol contributed to the accident. Police are looking for more witnesses. 2 Dead in Fatal Westminster Motorcycle Crash

Two people riding on a motorcycle collided into a truck Monday night in Westminster.

The accident took place on Edwards Street south of Industry Way. The two victims on the motorcycle were heading north. The motorcycle was a 2001 Ducati motorcycle. At around 9 p.m., an asphalt truck, attempting to turn left, collided into the motorcycle.  The traffic collision took place near the west side of Westminster Mall.

There was a male and a female on the motorcycle. Both riders were thrown from the motorcycle. One of the motorcycle riders was pronounced dead at the scene. The other motorcycle riders were taken to UCI Medical center in Orange with major injuries. The female pronounced dead at the scene. The male rider died a short time later at UCI Medical Center. He was in surgery at the time of his death. Names of the riders have not been released. The drive of the truck has not been identified either.

The driver of the truck remained at the scene and was not injured. Police have not determined who was at fault for the accident. Investigators are examining the possibility of speed contributing to the accident. Both directions of Edwards were closed as investigators were examining the scene.

Electrician Dies in Huntington Beach Fire

Brandon Orozco died Monday in a horrific accident in an underground vault in Huntington Harbour.

Orozco was part of a five member crew conducting scheduled maintenance. A fire broke out at around 12:15 in the underground electrical vault. The underground electrical vault is located in the 16200 block of Tisbury Circle near Mistral Drive. It is located on Humboldt Island.

The fire broke out when electricity suddenly began flowing through the fault. Orozco was the only member of the crew in the fault at the time. There were no other injuries reported.

Orozco’s body was removed from the vault at around 5:15 p.m. on Monday. When firefighters arrive to the scene, there was heavy smoke coming from the vault. It took the Huntington Beach and Anaheim fire departments five hours to enter the vault and retrieve Orozco’s body. After the fire was put out, the rescue workers had to wait for Edison to cut off power before entering the vault. Rescue workers had to test the vault air samples to make sure there were no more explosives in the area. The body was then transported to the Orange County coroner’s office.

The crew was from CAM Contractors Inc., a Southern California Edison subcontractor. The crew was contracted to work in the vault during a scheduled five-hour power outage. This is when the fire started. 350 customers were without power until 7 a.m. Tuesday.

Branzon Orozco was a union lineman for about three years. He worked for various electrical contractors. Orzoco had only been with CAM Contractors Inc., for about two months. His death was the first for CAM Contractors Inc.

Orozco is survived by his family. The Orozcos will have a viewing from 5 to 6 p.m. and a rosemary prayer service from 6 to 7 p.m. on Sunday at Rose Hills Memorial park in Whittier. On Monday morning, there will be a mass held at St. Andrew’s Catholic Church in Pasadena. The burial will be at Rose Hills on Tuesday. The cause of accident is still being investigated. Huntington Beach Fire Department investigators, Cal-OSHA, Edison, and CAM Contractors Inc. are working together to discover the cause of the fire.

Vehicle Kills Pedestrian at University High School

A pedestrian involved in an Irvine car accident near University High School succumbed to his injuries after being transported to a hospital on Monday.

Medicals respondents arrived to the site at approximately 4:30 p.m. to give a 78-year-old man who was struck by a car medical attention.

The pedestrian allegedly ran across the roadway on at Campus Drive and Paseo Montoya and was attempting to cross Campus at the time of the accident. It was during this time that he was struck by the vehicle, Orange County Fire Authority Capt. Steve Concialdi told reporters.

Concialdi said that the force of the impact threw the man “a good distance.”

He suffered serious injuries at the time of the Irvine car accident and had to be transported to UCI Medical Center in Orange, where he died from his injuries.

Officials want to notify the family of the victim before possibly releasing a name.

The accident remains under investigation. The driver operating the vehicle that hit the man remained at the scene and cooperated with authorities. No immediate arrests or citations were reported.

Newport Beach Motorcyclist Dies in Fatal crash

A male motorcyclist was killed early Saturday morning in a fatal motorcycle crash.

The motorcyclist has been identified as Ammar Sherbini, 31, of Santa Ana. He was riding his motorcycle at around 2:40. Sherbini was riding on West Coast Highway, west of Superior Avenue. Investigators believe he lost control of his motorcycle. Sherbini collided into narrow raised center divider. He was then thrown from his bike into a palm tree in the center divider. The bike came to a stop several hundred feet from the point of impact. No other vehicles or people were involved in the accident.

Sherbini was found dead at the scene. His injuries were quiet severe. Investigators said there was no initial evidence that drugs or alcohol were a factor in the crash. Excessive speed may have been a contributing factor, but investigators have not discovered the exact cause of the crash.

Police closed down all six of the southbound lanes of West Coast Highway. Only one northbound lane was left open. The Coast Highway was closed for several hours after the crash. It was not reopened by 7:30 a.m.  Investigators are still trying to determine the cause of the accident.

Irvine Attorney Arrested for Alleged Man Slaughter in Bicycle Accident

An Irvine attorney has been arrested in connection with a bicycle accident that occurred in March. She is being held under suspicion of manslaughter of the Rancho Santa Margarita man, who is remembered by friends as “caring.”

Attorney Hasti Fakhrai-Bayrooti, 40-years-old, was booked into county jail on Tuesday. Officials reported that she is suspect of being under the influence of prescription drugs at the time of the crash.

Orange County Sheriff’s Department investigators said that the Irvine attorney was behind the wheel of a 2003 Acura when she hit bicyclist Eric Billings along Santa Margarita Parkway on March 15. She allegedly drifted into the bicycle lane and slammed into Billings.

At the scene officials released Fakhrai-Bayrooti after the accident but pending blood test results. Sheriff Department’s Major Accident Reconstruction Team stayed behind to conduct the investigation.

After reviewing the result of the investigation, District Attorney’s Office has filed charges of vehicle manslaughter while intoxicated against Fakhrai-Bayrooti. After the arrest warrant was filed, she was taken into custody.

She was released Wednesday at 7:22 a.m. after posting bail.

5-Vehicle Crash Closed Southbound 5-Freeway

A five-vehicle crash forced officials to close the southbound lanes of the 5 Freeway Monday morning. They said that a motorcyclist was injured in the crash.

At approximately 10:35 a.m. California Highway Patrol received reports of the freeway crash and along with firefighters rushed to assist the injured.

The multi-vehicle wreck occurred at the Magnolia Street exit. It involved a motorcycle, two Honda Civics, a Nissan Altima and a Ford Taurus. CHP Officer G. Fithiam, told reporters that the rider of a BMW motorcycle was the only person injured and was taken to a hospital but the degree of the motorcyclist’s injuries were not disclosed.

Officials shut down the carpool lane, left lane, and off-ramp until 11:30 a.m. as CHP officers investigated the site.

There were no reports of arrests or intoxicated drivers, and the crash is still under investigation.

Laguna Beach Officer in Fatal Crash

Laguna Beach Officer was killed in a crash involving a pickup truck late Saturday.

Jon Coutchie was a 41 year old motorcycle officer. He was travelling northbound on South Coast Highway. Coutchie was trying to find a motorist who had been reported speeding.  Unfortunately, he was broadsided when a GMC pickup attempted to turn onto Cleo Street. The accident took place at around 11:30 p.m.

The GMC pickup truck was attempting to make a left turn. The driver collided into Coutchie’s Honda ST 1300. Coutchie was thrown from the motorcycle. Two officers reported to the scene of the accident. By the time the officers reported to the scene, Coutchie had already succumbed to his injuries.

The driver of the pickup was a 20 year old. The driver stayed at the scene of the accident. Investigators questioned the driver, and he was released. Police do not believe alcohol or drugs were factors in the crash. The California Highway Patrol is investigation the crash. It is not clear whether Jon Coutchie had his lights or siren on.

Coutchie is the first Laguna Beach officer to die in the line of duty since 1953. Before he was a police officer, he served as an U.S. Army Ranger. He was a ranger from 2003 to 2007. Coutchie joined the military after the 9/11 terrorist attacks. He was deployed to Iraq and Afghanistan, and he served two tours in Afghanistan and Iraq. Coutchie was given an honorable discharge.

He started working as a part-time jailer at the Laguna Beach Police Department. The Laguna Beach Police Department sponsored him through the Goldenwest Police Academy in 2008. He became an officer in 2009. Captain Jason Kravetz described Coutchie has a hard working officer who was passionate about his job.

Coutchie was well liked in the department. The department is a small department of 47 police officers and 40 non-sworn staff members. This death was a shock to the whole department. The department created a makeshift memorial of flowers, balloons, and an American flag were erected near the scene of the crash. Coutchie is survived by his parents, brother, stepsister, and girlfriend.  The accident took place 11 days before Coutchie’s 42nd birthday.

Alcohol May be Involved in Fatal Westminster Crash

A 32 year old Anaheim male was arrested on suspicion of felony DUI after he was involved in a fatal car crash early Tuesday.

The collision took place at McFadden Avenue and Brookhurst Street in Westminster. Police reported to the scene at around 1:11 a.m. A white Nissan Frontier truck and a red Honda Civic collided at the intersection. Police believe that the driver of the Nissan, Emmanuel Garibay, broadsided the Honda Civic. Investigators believe that Garibay was heading southbound on Brookhurst. The Honda Civic was heading westbound on McFadden Avenue. One of the vehicles ran a red light, and the vehicles collided.

There were no passengers in either vehicle. The driver of the Honda civic was fatally injured in the accident. He had to be cut out of the wreckage of the Honda. He was later identified as Joey Nungaray, 27, of Fountain Valley. Nungaray was taken to UCI Medical Center in Orange for treatment. He was later pronounced dead before 2 a.m.

Garibay also sustained injuries but minor ones. He was taken to UCI Medical Center to be treated. Garibay was then booked at the Orange County Jail on suspicion of drunken driving. The bail was set at $100,000. More details about Garibay’s arrests are not known.

Authorities are still working on investigating the crash and its cause. Investigators are still trying to determine which vehicle failed to stop at the red light. It is still not clear why one of the vehicles failed to stop.  Investigators do believe that alcohol was a factor in the collision.
